This was actually used!

Used in march 2000, for the work council (Ondernemingsraad) elections at XS4ALL.
Out of 97 people who could vote, 79 (81%) actually voted during the 3-day vote period.
The blind signature program runs on windows too, thanks to activestate!
Return of the anonymous votes was through an anonymous remailer.
Or, since remailers weren't reliable then, by abusing a badly configured open smtp relay.
Of those 79 votes, 76 were done by using "simplistic algorithm 1" (the password-protected CGI script).
Only 3 people used the blind signature protocol to get an anonymous vote number.
Only 2 anonymous votes were received back through email.
